COVID-19 Vaccination Update

Memorial Healthcare System continues to offer COVID-19 vaccines, following the Centers for Disease Control and Prevention (CDC) recommendations to administer the vaccines. 

At this time, Memorial's COVID-19 vaccine appointments for first doses have been filled.

As soon as we receive the next shipment of vaccines, we will reopen appointments via MyChart and our Centralized Scheduling phone number. Please return to this page for ongoing updates as we receive additional vaccines.

Who is eligible to receive the COVID-19 vaccines from Memorial Healthcare System when available??

  • Persons 12 and older who are Florida residents or seasonal residents, with proper identification as listed below, as approved by the Food and Drug Administration.
    Only the Pfizer COVID-19 vaccine is approved for 12 years and older.
  • Memorial's workforce and all active Memorial medical staff.

How do I register for an appointment?

MyChart:

  • If you have an existing MyChart account, login to your account.
  • If you do not have an existing MyChart account, sign up now.
  • If you are using the website version of MyChart, after logging in, choose "Visits" and then "Schedule an Appointment".
  • Choose a date, time and location that is convenient for you.

Second dose appointment

You can schedule your second dose appointment as soon as you receive your first dose. Register via MyChart or Call 954-276-4340 Monday — Friday 8 am to 5 pm. Please be advised of longer than usual wait times due to high volume of calls.

Remember to bring your vaccine card when you return for your second appointment.

Lost vaccine card

If you lost your card, you can view and print a record of your COVID-19 vaccination by following these steps:

  1. Login to your MyChart portal.
  2. Choose the Menu icon 
    Click on the Menu icon
  3. On the dropdown menu under My Record click on COVID-19
    in the Menu scroll to find COVID-19 under  the myrecord section
  4. You will find your vaccine records with Memorial under this section.

What should I bring to my vaccination appointment?

  • Adults must bring a valid Florida driver’s license or a valid Florida identification card.
  • If the person is under 18, the parent must bring proof of Florida residency and proof of the adolescent’s age
    • Birth certificate
    • Passport
    • State issued ID card, Driver’s Permit or Driver’s License
  • Seasonal residents who do not have a valid Florida driver’s license or valid Florida ID, must bring a photo ID in addition to appropriate residency documents listed below.
  • If you are coming for your second dose bring the vaccine card provided to you on your first dose appointment.
     

Effective January 22, 2021, in accordance with a public health advisory by Florida Surgeon General Dr. Scott A Rivkees, all persons seeking the first dose of a COVID-19 vaccine must show proof of Florida residency or must be in Florida for the purpose of providing health care services involving direct contact with patients.

To prove residency:

  1. An adult resident must provide the department with a copy of his or her valid Florida driver license issued under s. 322.18 or a copy of a valid Florida identification card issued under s. 322.051.
  2. An adult seasonal resident who cannot meet the requirements of subparagraph 1. may provide the department with a copy of two of the following that show proof of residential address:
    • A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
    • One proof of residential address from the seasonal resident’s* parent, step-parent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
    • A utility hookup or work order dated within 60 days before registration in the medical use registry.
    • A utility bill, not more than 2 months old.
    • Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
    • Mail from a federal, state, county or municipal government agency, not more than 2 months old.
    • Any other documentation that provides proof of residential address as determined by department rule.

*For the purposes of this paragraph, the term “seasonal resident” means any person who temporarily resides in this state for a period of at least 31 consecutive days in each calendar year, maintains a temporary residence in this state, returns to the state or jurisdiction of his or her residence at least one time during each calendar year, and is registered to vote or pays income tax in another state or jurisdiction.

PLEASE NOTE: No walk-ins will be accepted at the vaccination site. On-site registration is not available. You must register for an appointment before coming to the site. You can register via MyChart or by calling 954-276-4340.

What should I expect when I arrive for my appointment?

  • Vaccines will be provided only by appointment, while supplies last.
  • Wear a shirt or top with the upper arm accessible.
  • No pets allowed. Service animals, with documentation, allowed.
  • At your vaccine appointment, your temperature may be taken and you will be checked in. You will be given a vaccine fact sheet and asked a series of questions including any allergies.
  • Once you receive your vaccine, you will be observed for 15 to 30 minutes. You can use the time to schedule your follow up appointment for the second dose and learn about V-Safe, the CDC vaccine safety monitoring system.
  • When you get your first dose, you will get a vaccination card to show when you return for your second dose of COVID-19 vaccine. Remember to bring your card when you return.

Is there a cost for the vaccines?

Vaccines are provided at no cost to recipients and insurance is not required. However we will request consent to verify insurance coverage; no fees will be billed to the patient. Memorial is committed to administering the COVID-19 vaccine to individuals regardless of the vaccine recipient’s ability to pay COVID-19 vaccine administration fees. Other Vaccination Providers

The state of Florida has developed a pre-registration system to schedule COVID-19 vaccinations for priority populations. For more information visit myvaccine.fl.gov

Florida Department of Health in Broward County is also providing COVID-19 vaccination opportunities at the following Broward County vaccination sites.

  • Vaccinations will be available at no cost, by appointment only.
  • Appointments are required and can be made at BrowardCovidVaccine.com.
  • Due to an extremely large number of visitors to the site, the web page to schedule your appointment may not be immediately available.

The initial data and efficacy of the COVID-19 vaccines are encouraging, yet they are not the whole story. Equally as important as the vaccine is the practice of following safety guidelines: wearing a mask, practicing physical distancing and washing hands often.

Frequently Asked Questions

What vaccine is Memorial Healthcare System administering?

Memorial Healthcare System is currently using two COVID-19 vaccines:

  • the Pfizer-BioNTech COVID-19 vaccine is a two dose series separated by 21 days.
  • the Moderna COVID-19 vaccine is a two dose series separated by 28 days.

If you receive one dose of the Pfizer-BioNTech or Moderna COVID-19 vaccine, you should receive a second dose of the same vaccine at the proper interval to complete the vaccination series.

The vaccine is given in the deltoid arm muscle, similar to where a flu shot is given.

The U.S. Food and Drug Administration issued the emergency use authorization for this vaccine on December 11, 2020 for the prevention of the 2019 coronavirus disease or COVID-19 for individuals 16 years of age and older.

Can I choose which one of the vaccines I want to take for my first or second dose?

No. The first dose of the COVID-19 vaccine is administered depending on availability. The second dose will always be the same brand as the first dose.

What are the most commonly reported side effects?

The most commonly reported side effects, which typically lasted several days, were:

  • Chills
  • Fever
  • Headache
  • Joint pain
  • Muscle pain
  • Pain at the injection site
  • Tiredness

More people experienced these side effects after the second dose than after the first dose, so it is important for vaccine recipients to expect there may be some side effects after either dose.

If I have other medical conditions, should I tell the vaccine provider?

Tell your vaccine provider about all of your medical conditions, including if you have any allergies, have a fever, have a bleeding disorder or are on blood thinners, are immunocompromised or on a medicine that affects your immune system, are pregnant or plan to become pregnant, are breastfeeding, have received another COVID-19 vaccine.

Should I get the vaccine if I've had a severe allergic reaction to a vaccine previously?

You should not get the Pfizer-BioNTech or the Moderna COVID-19 vaccines if you had a severe allergic reaction after a previous dose of either one of these vaccines or had a severe allergic reaction to any ingredient of these vaccines.

Should I get the vaccine if I've been diagnosed with COVID-19?

If you have had COVID-19 in the past, you may receive the vaccine once you are out of your isolation period, which is commonly 10 days after your positive test or symptom onset.

  • If you have been exposed to COVID-19, you may receive the vaccine once you are out of your quarantine period, which is commonly 10 to 14 days after your last exposure.
  • If you have had COVID-19 in the past and received convalescent plasma or monoclonal antibodies, we recommend you delay your vaccine appointment for 90 days after treatment.
