Memorial Home Health Services is intended to help you recuperate in the comfort of your home through skilled nursing and rehabilitation services. Home Health must be ordered by your physician when he or she decides your condition requires you to remain in the home for rehabilitation and recovery.

As a partner in your care, the goal of Home Health is to teach and train the patient, family or caregiver on the patient’s care. We will ensure that you, or your designated representative, are competent, comfortable, and continue to provide care through the training process. Home Health is an intermittent service and is meant to be short-term.

What to Expect from Home Health Services

What is the difference between Home Health and personal home care assistance?

The difference between Home Health and personal home care assistance is the necessity for skilled services.

Home Health requires skilled services such as nursing, physical therapy, etc., that would come from a physician’s order. A home health aide may be part of the services provided as long as another skilled service is being provided and ordered by your physician. The Home Health visit involves personal grooming or other services assigned by the skilled professional.

Personal care assistance involves unskilled services typically paid for out of pocket (not paid by insurance). These services include help around the house, transportation, bathing and meal preparation. If you have long-term care insurance, many of these services are covered.

How often do the staff come to my home? How are these visits scheduled?

The frequency of your visits will be planned by your nurses and therapists, once they complete your evaluation and depending on your health status and needs. They will inform you of the frequency of visits at the end of the evaluation. Please share your doctor appointments and other commitments so your Home Health services can be planned more efficiently.

How long are the visits?

The visits will last as many minutes as necessary so our home healthcare professional can provide you with the best service. Evaluations usually take the most time. Your home healthcare professional will enter visit notes into your electronic health record on their computer and make phone calls to coordinate your services more efficiently. You are our priority, and all the activities that they carry out at your home are related to their services.

How long will I receive services?

Services continue until you achieve the goals of your care plan, as developed with you and your therapist or nurse and includes recommendations from your physician. They will share with you the content of your care plan and update you on every change, which must be approved by your doctor. It is your right to participate in the design of your care plan.

Remember, home health agencies are part of a network of health services that guarantee the continuity of your care. Once you finish with our services, you may be referred to other services according to your needs and with your approval. These services may include outpatient services, additional consult referrals or other community resources, in collaboration with the assistance of your doctor or simply the care of your family or your own care.

What procedures are in place to handle emergencies?

Our home healthcare agency has a 24/7 call service, which will contact the nurse on call to answer your clinical questions and help troubleshoot any problems with equipment, medication or an acute medical condition.

If immediate attention is needed, you will be prompted to call 911 or visit the nearest emergency room.

How often does the clinical team speak to my physician?

The clinical team speaks to your physician at various times during your home health admission:

  • at the start of your care,
  • when there are changes to your medical condition, and
  • when you are discharged from home health services.

Can you only get home care in a private residence?

No, Home Health can be provided in assisted-living facilities, group homes and shelters if permitted by the individual facility.

Will my insurance cover home healthcare services?

Home Health is covered 100% by Medicare, and we accept most types of insurance. Insurance verification is part of the admission process. Some insurance policies have copayments and deductibles. Please reach out to your insurance company for questions regarding your coverage.
